You have two issues. First, you have cellulite or dimpling throughout the buttock. Second, you have extra skin and fat on the inner thigh. To address the cellulite on the buttock, you could undergo VASER liposuction. However, the best way to correct cellulite is to transect the vertical bands that contribute to the cellulite. This can be achieved with the VASER. However, there is a better probe that can help to cut the vertical bands that cause cellulite. This instrument resembles a fondue fork with a blade between the tips. This helps to cut the vertical bands. This instrument is long so that each buttock can be treated from a single 5mm incision. Another perhaps better alternative and more reliable alternative than VASER to the buttock would be a posterior buttock lift. During a posterior buttock lift the incisions from your prior tummy tuck would be connected. An incision would be made across your lower back or on top of your upper buttocks. The incision would be connected just underneath your tattoo. An elliptical area of skin and fat of the upper buttock would be removed, and the butt would be lifted. This is an excellent way to eliminate cellulite and lift the loose skin of your buttocks. Most plastic surgeons would agree you would get the best result with a posterior body lift instead of VASER liposuction to the buttock cellulite. Regarding your inner thighs, you could undergo a crescent lift or a modified longitudinal vertical media thigh lift. More liposuction should be performed on the inner and posterior thighs during a modified thigh lift. During a crescent thigh lift, the incision is kept high in the thigh and just underneath the butt in the banana roll. During the crescent thigh lift, a crescent area of skin upper thigh skin and fat are removed. During a modified longitudinal, vertical medial thigh lift find, liposuction is performed, and an incision is made from the groin partially down the inner thigh.
